To apply for membership, you must complete the application form and submit a $25 fee. You can either use our Online Membership Form on this page and make your payment using the “Go To Payments” button, or you may download the form in either PDF or Word format, fill out the application, and mail it to us along with a check.
If you wish to further support SALVA, you may also make a donation exceeding the required $25. These donations greatly assist us in giving back to the community.
You can use the Online Membership Form here to submit an application for your business or organization. It is very important that you fill the Business Name field correctly, because this will be the main reference for your payment.
Use the green “Go to payments” button below to make a payment for the amount of the membership you wish to purchase.
PLEASE REMEMBER TO ADD YOUR NAME OR YOUR ORGANIZATION´S NAME TO THE “WRITE A NOTE” SECTION OF THE PAYMENT FORM (As shown in the example below) So we can associate your payment to the information on the application form.
